Additional Manufacturer Narrative
E1 - phone number: (b)(6).Investigation evaluation: the product said to be involved was returned in an open pouch from the lot number provided in the report. The label matches the product returned.A photo provided shows the complaint device coiled through a clear pouch. Our laboratory evaluation of the product said to be involved confirmed the report. The device was returned with the basket fully retracted and the handle was broken. The threaded distal end of the basket handle has cracked and separated from the handle with the threaded portion remaining seated in the luer adapter. The handle cannula has bent near the fracture. It is unknown when the breakage occurred as the handle appears to have been intact in the photo provided by the user. The drive wire was separated from the handle. There was nesting of the drive wire observed inside the purple hub.For further evaluation of the drive wire cable and basket, the catheter was cut to push the basket out of the sheath. The basket was fully formed and intact, and evidence of solder was present on the handle cannula at the joint.A product discrepancy or anomaly that could have contributed to this reported occurrence was not observed. The device history record for the lot number said to be involved was reviewed.A discrepancy or anomaly was not observed with the product that was released for distribution.Investigation conclusion: our laboratory evaluation confirmed the report of unable to manipulate the basket. This report was due to a separation of the device in the handle. The customer responses indicate that the scope used was an olympus gif-h190, which has an accessory channel of 2.8mm.The minimum channel size for the reported complaint device is 3.2mm.The use of an inappropriate scope is a likely cause for the reported occurrence.Additionally, the instructions for use states: "confirm the desired position of the basket sheath relative to the target.Advance the basket out of the sheath by pushing forward on the handle.Caution: pulling on the sheath while advancing or retracting the basket may damage the device, rendering it inoperable." the instructions for use also indicates: "advance the device through the channel, in short increments, until the basket sheath exits the endoscope." basket deployment difficulties and buckling/breaking of the drive wire can occur if the device experiences excessive pressure.Resistance in basket movement and bends in the catheter can occur if the elevator of the endoscope is used to deflect the device at a sharp angle.Prior to distribution, all memory ii double lumen extraction baskets are subjected to a visual inspection and functional testing to ensure device integrity.A review of the device history record confirmed that the lot said to be involved met all manufacturing requirements prior to shipment.Corrective action: based on the quality engineering risk assessment no corrective action is warranted at this time.A review of the complaint history was conducted.Quality assurance will continue to monitor for complaint trends.Additional comments: based on the information provided, the scope used was an olympus gif-h190, which has an accessory channel of 2.8mm.The minimum channel size for the reported complaint device is 3.2mm.The use of an inappropriate scope is a likely cause for the reported occurrence, therefore a cook representative has been directed to contact the medical facility involved in an effort to promote further education and understanding related to appropriate usage of this product.
