H11: section a through f - the information provided by bd represents all of the known information at this time.Despite good faith efforts to obtain additional information, the complainant / reporter was unable or unwilling to provide any further patient, product, or procedural details to bd.The complaint of air leakage through the catheter is inconclusive because the failure could not be replicated. One 15 cm trialysis catheter was returned for evaluation. An initial visual observation of the returned catheter showed blood residues throughout the returned catheter.A functional test of attempting to infuse water into each lumen of the catheter using a 12 ml syringe revealed the catheter to be patent to infusion with no observable leaks.A functional test of attempting to aspirate water through each of the lumens using a 12 ml syringe revealed the catheter aspirated water without observable air leakage.A functional test of pressurizing each lumen with water revealed no observable leaks throughout the catheter; however, due to the large french size of the catheter, it could not be determined whether pressurizing the catheter correctly simulated complete blockage of the distal end of each lumen of the catheter.A microscopic observation revealed nothing remarkable throughout the catheter.Because exact clinical conditions could not be replicated, the complaint of air leakage through the catheter is inconclusive at this time.Possible contributing factors may include luer connection to their respective devices or other unknown conditions. This complaint will be recorded for future trending and monitoring purposes.
