H3 evaluation summary: medtronic conducted an investigation based upon all information received. One opened device and eight sealed representative samples were available for evaluation.Visual inspection of the opened sample noted one suture and one needle. The needle was returned detached from the suture.Microscopic inspection of the needle showed normal crimp and swage marks. The swage contained suture material, indicating the suture had broken at the swage. The foil pouch was inspected, and no signs of damage or abnormalities were observed.Visual inspection of the packaging of the representative samples showed no damage or breaches. The seals were inspected and found to be closed. After opening the packaging, the needles were properly placed in the retainer foam, and the sutures were wound inside.Functional testing of the seven representative sutures showed that the breathable pouches were opened without tearing the packaging.Six of the sutures were removed from the retainers without difficulty. The sutures were secured in a testing device, and no breakage or fraying occurred during handling or setup. The sutures were then pulled until the needles detached. The force at the point of detachment was measured and met the applicable requirements. The seventh sample detached prematurely while still in the retainer.Microscopic inspection showed that the needle swage was partially filled with suture material. It was reported that the needle detached. The reported issue was confirmed. The most likely cause could not be established from the information available. The evaluation detected an unreported condition: needle prematurely detached in retainer. The most likely cause could not be established from the information available. The manufacturing records for each device are thoroughly reviewed prior to release to ensure that it meets all medtronic quality specifications.A secondary review of the device history records found no potentially contributing factors.Medtronic submits this report to comply with fda regulations 21 cfr parts 4 and 803.Medtronic has made reasonable efforts to provide as much relevant information as is available to the company as of the submission date of this report. This report does not constitute an admission or a conclusion by fda, medtronic, or its employees that the device, medtronic, or its employee caused or contributed to the event described in the report.Any required fields that are unpopulated are blank because the information is currently unknown or unavailable.Medtronic will submit a supplemental report if additional relevant information becomes known.
