Event or Problem Description
According to the reporter, during an ileocecal resection, when the surgeon used the stapler, it malfunctioned.The surgeon squeezed the handle; it clicked into place, and the specimen was cut, but when the stapler was being removed, the staples were only partially deployed.A new staple was deployed to resolve the issue.
