Event or Problem Description
As reported, upon opening the package of the filiform double pigtail ureteral stent set, it was discovered that one side of the tip was broken.The break was found prior to an ureteroscopy and laser stone fragmentation (ursl) procedure and did not make patient contact.The procedure was completed by using another-same like device.A customer provided photo appears to show a curl of the stent is missing, and the tether is absent.As reported, the patient did not experience any adverse effects or require any additional procedures due to this occurrence.
