Additional Manufacturer Narrative
H11: manufacturing review: a manufacturing review was not required as this is the only complaint reported to date for this product and lot.Investigation summary: three photos were provided and reviewed. The first photo shows distal end of the balloon with the tip detached and blood residues were noted throughout the detached part. The second image shows the labeling details of the device that match with the information available in trackwise and the third photo shows the vaccess device identified a circumferential rupture in the balloon with blood residues present throughout the balloon. No additional anomalies were observed. Therefore, the investigation is inconclusive for the reported entrapment as reported condition of the issue could not be replicated in the laboratory. However, the investigation is confirmed for the reported detachment as the balloon with the tip was detached and the investigation is identified and confirmed for circumferential balloon rupture based on photos review.A definitive root cause for the reported detachment, entrapment of device and the identified circumferential balloon rupture could not be determined based upon the provided information.Labeling review: as the reported event did not allege a labeling or use related issue, a labeling review is not required.D2b: (dqy; lit).G3, h6 (device, component, method, result, conclusion).Section a through f: the information provided by bd represents all the known information at this time.Despite good faith efforts to obtain additional information, the complainant / reporter was unable or unwilling to provide any further patient, product, or procedural details to bd.
