Event or Problem Description
It was reported that the surgeon had completed a latarjet procedure and was using a q-fix knotless anchor to complete the labral repair. The surgeon was attempting to drill into the glenoid with the q-fix drill, 3-4 attempts were made as each time he drilled and tried to place the anchor but it was not possible to get the anchor flush with the drill guide. On the last attempt, when the surgeon pulled the drill out of the drill guide, he noted that around 2cm of the drill had snapped off inside the patients glenoid.Attempts were made to retrieve the drill but as it was flushed with the bone it could not be pulled out.The piece of the drill was left in the patient. The procedure was completed using a back-up device. There was no delay and no further complications were reported.

Additional Manufacturer Narrative
H11: h3, h6: the reported device was received for evaluation.A visual evaluation showed the device was not returned in original packaging. The distal tip of the drill is fractured away and was not returned. There is no other visible defect.Bio debris is present.A review of device records showed there were no indications to suggest that the product did not meet manufacturing specifications upon release for distribution.A complaint history review found no similar reported events. The instructions for use were reviewed and found to include conditions of off label use and technique specifics, as well as precautions and warnings related to the use of the device.A risk management review found that the reported failure and/or harm was documented appropriately, and there were no indications to suggest the anticipated risk is not adequate.A definitive root cause for the reported issue could not be determined.Factors that could have contributed to the failure include procedural variance, or deviations from established protocols, increasing risks and leading to unintended consequences (excessive load applied to the drill while in use, incorrect attachment of the drill to the drill handle or an inadvertent impact event inconsistent with normal use).Based on this investigation, the need for corrective action is not indicated.
