It was reported that during a da vinci-assisted radical without lymphadenectomy prostatectomy surgical procedure, a nurse contacted technical support to report that error 25326 occurred. The intuitive surgical, inc.(isi) technical support engineer (tse) guided the customer to check and power cycle the system, but the issue persisted.Subsequently, the tse suggested to swap the patient side cart (psc) power socket, but the problem continued.Due to these unresolved issues, the procedure could not be completed as planned. No known impact or patient consequence was reported.Intuitive surgical, inc.(isi) followed up with the initial reporter and obtained the following additional information: the customer elected to convert to traditional laparoscopic surgery after the da vinci system went down. There were no increases in port incisions, nor were additional ports placed. The patient tolerated the conversion well, and there was no injury to the patient as a result of the event.

Intuitive surgical, inc.(isi) field service engineer (fse) was dispatched to the customer site to further investigate the reported event. The fse was able to reproduce the customer-reported issue, noting the battery indicator flickering with the start button off, while the power cord and outlet were confirmed normal.Initial power cycles showed intermittent behavior, and the system shut down with error 23.In consultation with technical support, potential defective components were identified as the battery, auxiliary power board (apb), and generic power distributor (gpd). The battery was replaced, but the issue persisted. Then fse swapped the gpd board using the surgeon side console (ssc) gpd, after which the patient side cart (psc) functioned normally. The system was power cycled five times without recurrence of the issue. The system was tested and verified as ready for use.Isi did receive a da vinci product involved with this complaint to perform failure analysis. The gpd was analyzed, and failure analysis investigations replicated and confirmed the customer-reported complaint. The system log review identified error 25326, confirming the issue occurred in the field.Visual inspection revealed no abnormalities related to the reported issue. The gpd was installed in the golden system, where it failed to distribute power to the ssc.During testing, flashing leds were observed on the board, indicating a functional failure.Upon completion of testing, system error logs were reviewed, and failure analysis was able to confirm and replicate the reported event. The complaint was confirmed based on the field evaluation and failure analysis. The probable cause of error 25326 is a faulty gpd board, which resulted in a power disconnection to the system.
