Event or Problem Description
It was reported that, during a remplissage with instability surgery, it was noticed that the helicoil anchor sa pk 4.5mm got broken, when the surgeon screwed the suture anchor in the bone. The procedure was resumed, after a delay less than 30 minutes, using a similar s+n back-up product. No further complications were reported.

Additional Manufacturer Narrative
H11: internal complaint reference: (b)(4).H3, h6: the reported device was not returned to the designated complaint unit for independent evaluation.A complaint history review found similar reported events.A risk management review found that the reported failure and/or harm was documented appropriately, and there were no indications to suggest the anticipated risk is not adequate.A review of the raw material requirement found that the tensile strength has been established. Also, material certificate of analysis (coa) is required for raw material.Although a review of device records showed there were no indications to suggest that the product did not meet specification upon release for distribution at the time of manufacturing, the root cause of the reported event has been traced to a manufacturing issue.Based on this investigation, a nonconformance was previously initiated to address the reported failure. Therefore, the need for corrective or preventive actions is not indicated.
