Block h6: imdrf device code a0501 captures the reportable event of cautery pin detached.Block h11: the captivator emr device was returned and during visual inspection, it was observed that the working length and wire were kinked at the proximal section (strain relief).Additionally, the device was returned with the cautery pin broken/detached.It is important to mention that the upper part of the cautery pin was not returned for analysis. No other problems with the device were noted. The reported event of cautery pin detached was confirmed.Investigation found that the working length and the wire were kinked at the proximal section (strain relief).Additionally, the cautery pin was detached. These problems likely have occurred due the manner as the device was handled and manipulated may have caused the reported and encountered problems.Excessive manipulation of the device without enough care could have induced the defects found. Also, this issue could have occurred due to the way the active cord was connected or disconnected from the cautery pin (2-in-1 connector), which may have caused the cautery pin to become broken/detached, however, since the upper part of the cautery pin was not retuned for analysis, it is not possible to determine the definitive cause for this issue. Therefore, the most probable root cause is cause not established.
