Event or Problem Description
According to the reporter, during a procedure, the suture thread comes off the needle every time the needle crosses the abdominal wall, the issue happened with a different suture belonging to the same package then a third needle breaks halfway through abdominal wall.A new suture was used to resolve the issue.There was no patient injury.
