Additional Manufacturer Narrative
H11: section a through f - the information provided by bd represents all of the known information at this time.Despite good faith efforts to obtain additional information, the complainant / reporter was unable or unwilling to provide any further patient, product, or procedural details to bd.The complaint of a split in the catheter is confirmed, but the root cause could not be determined. One 20 ga powerglide pro catheter with extension tubing attached was returned for evaluation. An initial visual observation of the returned catheter showed blood residues throughout device.A circumferential split was observed just distal to the luer. The catheter was returned attached to extension tubing.A functional test of flushing the catheter with water using a 12 ml syringe revealed a leak just distal of the molded hub.Blood residues were flushed to better observe the split. No obvious blood occlusion was observed during functional testing of the device.Microscopic inspection of the returned catheter shaft showed an ¿l¿ shaped split just distal of the molded hub.One corner of the split was observed to have characteristics of stretching or pulling of the catheter, leading to a split in the catheter. The opposite corner of the split had sharp fracture edges, indicating a separate cause of failure to initiate the split. The root cause of the failure of the catheter shaft could not be determined; however, potential root causes may include contact of the catheter shaft with a needle or sharp instrument, damage to the catheter during insertion procedures, damage to the catheter during the manufacture of the device, or damage caused by other clinical factors. This complaint will be recorded for future trending and monitoring purposes.
