Event or Problem Description
It was reported the gastrointestinal videoscope had a burnt distal tip.The issue occurred during an unknown procedure.The procedure was completed with a similar device.There were no reports of patient harm.

This supplemental report is being submitted to provide the results of the final investigation.The device was returned to olympus for inspection, and the reported failure was confirmed.Based on the results of the investigation, the most likely cause of the burned cover was unable to be determined.Should additional relevant information become available, a supplemental report will be submitted.Olympus will continue to monitor field performance for this device.
