Additional Manufacturer Narrative
Product complaint # (b)(4).Investigation summary: defective cup in failure of placement (imd).During an intermediate hip prosthesis, the definitive acetabular implant does not clip correctly; the surgeon requests another implant. The product was returned to depuy synthes for evaluation.Visual inspection of the returned device found nothing indicative of a device nonconformance that would contribute to the reported complaint allegation. The evaluation of the returned device, as well as previous analysis performed of related complaints (b)(4), found that the observed slight toggle while the collar and liner are locked inside the cup is acceptable by design and would not contribute to premature failure of the components.Although there was slight movement with thumb pressure between the mating components, the components locked together correctly, and performed as design intended. Therefore, the alleged "mating parts do not fit together", is not confirmed and there is no manufacturing or design related product issues found of the mating components in the condition they were received.A manufacturing record evaluation was performed for the finished device product code: 103545000 lot number: d25061013, and no non-conformances or manufacturing irregularities were identified. The overall complaint was not confirmed as the observed condition of the self cent hip 45x28 gry would have not contributed to the complained device issue.Based on the investigation findings, it has been determined that no corrective and/or preventative action is proposed. There is no indication that a design or manufacturing issue has caused the reported complaint condition. As part of depuy synthes quality process, all devices are manufactured, inspected, and released to approved specifications.Additional monitoring for any potential safety signals will be conducted through complaint trending and other post-market safety surveillance activities.Device history lot: a manufacturing record evaluation was performed for the finished device product code: 103545000 lot number: d25061013, and no non-conformances or manufacturing irregularities were identified.Device history review: a manufacturing record evaluation was performed for the finished device product code: 103545000 lot number: d25061013, and no non-conformances or manufacturing irregularities were identified.Corrected: h3.
