It was reported that a patient underwent a cardiac ablation procedure with preface sheath and the cap fell off issue was observed. The device evaluation was completed on 24 march 2026.The product was returned to johnson & johnson medtech for evaluation. It was sent to the device manufacturer for further investigation.Visual and dimensional analysis of the returned device were performed following johnson & johnson medtech procedures.A non-sterile unit of a preface sheath was received inside a clear plastic bag.The device was unpacked to proceed with the product evaluation.During the visual inspection, the sheath with the hemostasis valve attached and catheter were received for analysis. The components were thoroughly inspected, and no anomalies were observed during the analysis. No apparent damage was noted on the hemostasis valve.A flushing test was performed in which water was introduced through the side port tubing, flowed through the hub, and exited at the cannula tip.No leakage was observed at the valve or hub during the test.Additionally, no apparent damage was identified on the hemostasis valve. The connector/valve assembly was securely attached to the hub, with no evidence of looseness or separation. No damage or separations were observed in the valve during the microscope analysis. The hemostasis valve was carefully examined under magnification, and all inspected surfaces appeared intact and properly bonded. There was no evidence of separation.A device history record evaluation was performed and no internal actions related to the complaint were found during the review. The complaint reported by the customer could not be confirmed. The analysis performed (visual, functional, and microscopic) did not reveal any abnormalities or conditions that could indicate a product defect. The product analysis did not provide evidence to suggest that the failure was related to the manufacturing or design process. As part of johnson & johnson medtech quality process, all devices are manufactured, inspected, and released to approved specifications. If additional information is received regarding this event, a supplemental 3500a report will be submitted to the fda.Manufacturer's reference number: (b)(4).
