Event or Problem Description
It was reported that an over infusion of medication occurred during the use of a clearlink secondary medication set with duo-vent spike.It was observed that a cefepime bag was completely infused faster than the expected therapy time.This issue occurred during patient infusion.There was no report of patient injury or medical intervention associated with this event.No additional information is available.
