Updated fields: d4, d9, g3, g6, h2, h3, h4, h6, h11.The accudrain without the anti-reflux valve (ins8400) was not returned; therefore, failure analysis is not possible. However, investigation was performed using phots provided by the customer as follows: device history record (dhr) review - the dhr was reviewed and no anomaly was observed that could be related to the reported condition.Failure analysis - the customer provided a photo of the device in which a drop is falling from the stopcock open side. However, the stopcock is open towards the patient side; therefore, it is expected to see csf flow.Another photo was provided after placing a blue cap to prevent it from dripping. The complaint cannot be confirmed from the photos provided since the stopcock is open to patient.Root cause ¿ based on the photos provided, it is concluded that the most probable cause for the reported leak is due to inadequate stopcock handling by the final user. The device is supplied with a red cap on the stopcock side where a blue cap was placed by the customer. It is not recommended to leave the stopcock connection site open to air to help prevent contamination. As per product ifu: ¿strict aseptic technique should be used during initial set-up, at all stages of utilization and maintenance, and any time the system must be accessed, changed, or otherwise manipulated in any way that opens the closed system once it is connected to the patient. If monitoring is not used, it is imperative to keep the red end cap located on the panel mount icp monitoring port securely tightened.¿ the last sentence is highlighted in bold letters in the ifu literature. In addition, the stopcock in the photos is the zero-reference stopcock (¿four way stopcock¿).The device contains two (2) stopcocks (the other one is on the patient line proximal to patient). These stopcocks are 100% inspected prior to assembly process. These stopcocks are received with a certificate of conformance and prior to assembly are verified 100% for cracks and stress marks under a microscope 3x (including the white plug). No capa (corrective and preventive action) escalation is required based on the acceptability of risk and no adverse trends were identified. This will be monitored and trended going forward. At present, we consider this complaint to be closed.
