Event or Problem Description
It was learned through implant patient registry that a patient with a 25 mm 11060a aortic valve was explanted after an implant duration of 2 years, 10 months due to unknown reason. The explanted valve was replaced with a 25 mm 11060a valve.Edwards lifesciences maintains an implant patient registry. This registry is a patient tracking mechanism for serialized edwards implantable devices (bioprosthetic heart valves and annuloplasty rings), rather than a true post-market surveillance registry.Through the registry, edwards is notified when these devices are implanted. In addition, patient and/or device status may be reported to the registry via the implantation data cards. The information is received from various sources (e.G.Surgeon, hospital, and patient family members) and is not received in the form of a conventional customer complaint. The information reported may or may not be related to the edwards device.

Event or Problem Description
It was learned through implant patient registry and investigation that a patient with a 25mm 11060a aortic valve was explanted after an implant duration of 2 years, 10 months due to endocarditis (c.Acne) with mycotic pseudoaneurysm and root dehiscence. The explanted valve was replaced with a 25mm 11060a valve.Per medical records, patient was admitted on (b)(6) 2025 with recurrent fevers and chills.A positive blood culture showed gram positive bacilli and patient was started on iv antibiotics.Cardiac imaging also showed mycotic pseudoaneurysm and root dehiscence, with endocarditis involving the noncoronary leaflet of his bioprosthetic aortic valve. The patient underwent redo-avr and root replacement with a 25mm 11060a valved conduit, debridement of aortic root due to infection, and removal of prior ppm.Postoperatively, the patient had ppm placed on pod#6.The patient was discharged to home on pod #19.Pathology report showed explanted prosthetic valve with reactive fibrous tissue with mixed inflammation and associated foreign material.
