Event or Problem Description
The following was reported to gore: on (b)(6) 2025, a patient was treated for an abdominal aortic aneurysm using a gore® excluder® conformable aaa endoprosthesis. The device was deployed; and then the device was reconstrained to push a little more proximally. However, when the physician tried to unconstrain, the device would not unconstrain, even though the black nut was all the way forward. The physician tried turning the nut towards reconstrained and unconstrained again; however, it remained constrained. The physician proceeded with the procedure, cannulating the contralateral gate using a gore® molding & occlusion balloon for confirmation.Prior to final deployment of the proximal end, the mob was taken proximally to the constrained end and inflated. This opened the device more, but not to the normal diameter of a partially deployed stent. The proximal end was then deployed to 100%, and the device did open fully. With further ballooning, the physician was able to create wall apposition. An aortic extender device was implanted to create more seal. The procedure completed successfully, and the patient tolerated the procedure.
