Event or Problem Description
It was reported the needle tip of the vizishot2 single use aspiration needle was broken off at the distal end.The issue was found during a diagnostic bronchoscopy procedure that was completed with a similar device.There were no reports of patient harm.
Additional Manufacturer Narrative
The device was returned to olympus for evaluation/inspection.Based on the results of the investigation, the needle was broken off at the distal end was identified.It is likely the result of fatigue; however, a definitive root cause could not be established.Should additional relevant information become available, a supplemental report will be submitted.Olympus will continue to monitor field performance for this device.
