It was reported the physician selected a 25mm gore® cardioform septal occluder to treat a patent foramen ovale.During deployment the occluder formed improperly and it appeared to be deployed in the tunnel. The physician reloaded the occluder, recrossed the septum, then deployed and locked the device.Upon further assessment, it was noted that the occluder was too small as it didn't capture the whole septum. The occluder was removed without issue; however, following removal a pericardial effusion was noted on intracardiac echocardiography.Pericardiocentesis was performed and the patient was stable. The procedure was concluded without defect closure.
