It was reported that a patient underwent a carotid endarterectomy on (b)(6) 2025 and suture was used.A highly experienced vascular surgeon, used article 8706 for carotid sutures.Unfortunately, they couldn't give me a specific date for the surgery. He's been complaining for months that the needles keep coming loose from the thread, sometimes even during removal from the wound support. All previous complaints have been inconclusive, which annoys him greatly. The doctor a highly experienced and accomplished vascular surgeon and has always been very satisfied with our sutures. There were no patient consequences reported.Additional information was requested.

Product complaint # (b)(4).This report is being submitted pursuant to the provisions of 21 cfr, part 803.This report may be based on information which has not been investigated or verified prior to the required reporting date. This report does not reflect a conclusion by ethicon inc, or its employees that the report constitutes an admission that the product, ethicon inc or its employees caused or contributed to the potential event described in this report. If information is obtained that was not available for the initial report, a follow-up report will be filed as appropriate.Additional information: h6, d9, h3.H3 investigation summary: the product was returned to ethicon for evaluation. One small needle- suture piece was received for analysis.Product code 8706h.The product code is double armed.During the initial inspection of the sample, no needle pulled off was noted. The swage and attachment area were noted to be as expected; however, marks that appear to be caused by a surgical instrument were observed on the body needle. The suture piece was inspected, and the extreme was noted to be cut and damaged (crushed) on the surface probably caused by a surgical instrument.Body fluids were also observed on the suture. The other section of the suture was not returned for analysis. The functional test was not possible because the suture was received with a short length. The instructions for use do contain the following caution: as with any device, care should be taken to avoid damage to the strand when handling.Avoid the crushing or crimping action of surgical instruments, such as needle holders and forceps. As part of our quality process, the manufacturing records of this lot-serial number were reviewed, and the manufacturing standards were met prior to the release of this batch.
