As reported, the balloon on a 3.5mm x 15cm saber.018 percutaneous transluminal angioplasty (pta) balloon catheter ruptured at 8 atmospheres (atm).A 3mm x 30cm saber.018 pta balloon catheter was then used for successful dilation, followed by the use of a non-cordis drug-coated balloon (dcb). There were no reported injuries to the patient. This was during a procedure to perform recanalization of a lower limb arterial occlusion. The target vessel was the femoral-popliteal artery at the p1 segment. The target vessel exhibited severe calcification, no tortuosity, and 80% stenosis. The device was stored and prepared according to the instructions for use (ifu). There was no difficulty removing the sterile packaging components or the protective balloon cover. The device maintained negative pressure during preparation. The device was not placed into an acute bend and was able to cross the lesion without kinking. The device was removed easily from the patient. The device will be returned for evaluation.

After further review of additional information received the following sections have been updated accordingly: b4, g3, g6, h1, h2, h3, h6, and h11.Complaint conclusion: as reported, the balloon on a 3.5mm x 15cm saber.018 percutaneous transluminal angioplasty (pta) balloon catheter ruptured at 8 atmospheres (atm).A 3mm x 30cm saber.018 pta balloon catheter was then used for successful dilation, followed by the use of a non-cordis drug-coated balloon (dcb). The device was removed easily from the patient.A non-sterile unit of ¿saber 3.5mm15cm 150¿ was received for analysis coiled inside of a clear plastic bag.The device was unpacked to proceed with the product evaluation.A thorough inspection was performed observing no damage or anomalies. The balloon was returned not inflated. No other outstanding details were observed.A functional test was performed. One inflator/deflator device was attached to the inflation port.Balloon inflation test was done applying positive pressure using the inflator/deflator device with the purpose of reach the rate burst pressure. However, inflation pressure is not maintained due to the leaking condition observed on the balloon. The balloon was inspected with a vision system observing a rupture on the surface where the water is leaking. The balloon surface presented a rupture condition with secondary scratch marks located near the damaged border area. This type of damage is commonly caused during the interaction of the material with a sharp object or mechanical damage. It is very likely that the same factors that caused the observed scratch marks on the surface could probably lead to the damaged condition found on the received device. It seems that the material near the damaged area was affected by a sharp object from the outside of the device. The reported ¿balloon burst at/below rbp¿ was observed as there was a rupture noted on the balloon surface. The exact cause could not be determined. The reported severe calcium of the femoral-popliteal artery may have contributed to the reported event; it is likely to have caused damage to the balloon material that led to its rupture.According to the warnings in the safety information in the instructions for use, ¿prior to angioplasty, the catheter should be examined to verify functionality and integrity, and ensure that its size and shape are suitable for the specific procedure for which it is to be used.Do not use if product damage is suspected or evident. To reduce the potential for vessel damage or the risk of dislodgement of particles it is very important that the inflated diameter of the balloon should approximate the diameter of the vessel just proximal and distal to the lesion. The balloon dimensions are printed on the product label. The compliance table incorporated with the product shows how balloon diameter increases as pressure increases.Do not exceed the rated burst pressure recommended on the label. The rated burst pressure is based on the results of in vitro testing. At least 99.9% of the balloons (with a 95% confidence) will not burst at or below their rated burst pressure.Use of a pressure monitoring device is recommended to prevent over-pressurization.Pressure in excess of the rated burst pressure can cause balloon rupture and potential inability to withdraw the catheter through the introducer sheath.Balloon rupture can cause vessel damage and the need for additional intervention.Use only the recommended balloon inflation medium (a 50/50 mixture by volume of contrast medium and normal saline).Never use air or any gaseous medium to inflate the balloon.¿ the information available does not suggest a design or manufacturing related cause for the reported event. Therefore, no corrective or preventive action will be taken at this time.
