Event or Problem Description
It was reported that a patient underwent a primary breast reconstruction procedure with a mentor memorygel xtra breast implant 775cc gel breast prosthesis that ruptured post implantation.Rupture of the patient¿s right breast prosthesis was observed during an implant exchange surgery on (b)(6) 2025.The explanted breast prosthesis was replaced with a new mentor memorygel xtra breast implant 775cc gel breast prosthesis.

Additional Manufacturer Narrative
On 25-dec-2025, mentor completed the investigation on the suspect medical device.Device evaluation summary: according to the information received, the patient experienced a rupture in the breast implant. The product was returned to mentor for evaluation.Mentor then conducted visual inspection, leak testing and microscopic examination of the returned device.Visual analysis of the returned device determined that the breast implant was found to have a tear in the anterior aspect with a section of missing material on the edge of the tear, measuring approximately 0.4cm and 0.1cm x 0.1cm respectively.Additionally, the breast implant appears white.Leak testing was performed, in accordance with mentor procedures, and no additional areas of gel exposure were detected during the analysis.Microscopic examination was performed on the edges of the rupture, and parallel striations were found in an area of the tear on the anterior aspect, measuring approximately 0.2 cm.Parallel striations are consistent with markings made by a sharp object perforating the implant shell. The cause of the rupture in the remaining area of the tear could not be identified.Mentor also performed a manufacturing record evaluation related to the reported complaint for the finished device lot number. No manufacturing non-conformances were identified as part of this evaluation.Discoloration of the implants as observed in the samples returned is related to the concentration of the triglycerides in the gel fillers and their degree of unsaturation. This finding is consistent with the reported discoloration of breast implant silicone gel in vivo in the scientific literature. As part of mentor¿s quality process, all devices are manufactured, inspected, and released according to approved specifications.Based on the results of this investigation, it was determined that the product met the manufacturing release criteria: no corrective and preventive action (capa) is required now.Additional complaint information monitoring for potential safety signals is conducted through complaint trending as part of post market surveillance.D6 investigation conclusions d17: cosmetic, explant discoloration.Manufacturer¿s reference number: (b)(4).
