Correct field d4 lot and udi number.Updated field b4, d4 expiry date, d9, g3, g6, h2, h3, h4, h6 type of investigation findings.Investigation conclusions, h11.A 40cc balloon was inserted under angiographic guidance and difficulty was immediately noted when advancing the 0018 guidewire through the balloon. After the guidewire was removed the clinician attempted to aspirate and flush the balloon but no fluid returned through the wash line aspiration was very difficult and infusion nearly impossible. After consulting with the physician the team reinserted the guidewire with difficulty but the lumen remained nonpatent once the guidewire was removed again. The decision was made to replace the balloon. The second balloon allowed smooth guidewire advancement and proper positioning though some resistance during aspiration and infusion persisted nevertheless therapy proceeded successfully. The balloon functioned adequately throughout treatment and was later removed after bypass surgery with the patient continuing on impella support. No patient harm was associated with the balloon issue.Postprocedure testing of the first balloon showed that flushing was possible but aspiration was still blocked. Both balloons were ultimately discarded. No decon or visual inspection performed since product was not returned and could not be evaluated. Therefore we were unable to confirm or determine a root cause for the reported failure. The failure mode is addressed in the risk file and is operating within its risk profile. The ifu addresses the reported failure. There were no ncmrs identified which could cause or contribute to the reported failure. The investigation does not indicate that the device was inadvertently released as nonconforming or an adulterated product or was a counterfeit. The complaint history review did not identify an adverse trend increase in number of complaints over past three 3 months.Based on the rational provided above no escalation to the capa process is required.
