Additional Manufacturer Narrative
Product complaint#: (b)(4).Investigation summary: according to the information received, ¿patient had surgery 1 year ago and had to be re-intervened because the polyethylene insert split. The pinnacle cup and polyethylene insert were removed".The product returned to depuy synthes for evaluation. The depuy synthes team conducted a visual inspection of the returned device.Visual analysis revealed that the delta cer head 12/14 28mm +5 exhibits signs of material transfer on its outer surface, most likely as a consequence of the reported liner disassociation, which caused the femoral head to articulate directly with the metal cup.The observed condition of the femoral does not represent a device no-conformance.A dimensional inspection and functional testing were not performed as it is not applicable to the complaint condition.A manufacturing record evaluation was performed for the finished device [lot.9699345] number, and no non-conformance's / manufacturing irregularities were identified during manufacturing. The overall complaint was not confirmed as the observed condition of the delta cer head 12/14 28mm +5 would not contribute to the complained device issue.Based on the investigation findings, it has been determined that no corrective and/or preventative action is proposed. There is no indication that a design or manufacturing issue has caused the reported complaint condition. As part of depuy synthes quality process, all devices are manufactured, inspected, and released to approved specifications.Additional monitoring for any potential safety signals will be conducted through complaint trending and other post-market safety surveillance activities.Device history lot: a manufacturing record evaluation was performed for the finished device [lot.9699345] number, and no non-conformance's / manufacturing irregularities were identified during manufacturing.Device history review: a manufacturing record evaluation was performed for the finished device [lot.9699345] number, and no non-conformance's / manufacturing irregularities were identified during manufacturing.Corrected: h3.
