It was reported that the patient presented with shortness of breath. On interrogation it was found that the right ventricular (rv) lead exhibited a warning for low impedance.Noise and diminished sensing was also noted.Undersensing was observed causing inappropriate pacing. It was also reported that there was unstable thresholds with intermittent capture and loss of capture.Oversensing of short v-v intervals was also noted.Insulation breach was suspected. It was further reported that the implantable pulse generator (ipg) was suspected for a pacing issue.Chest x-ray was completed. Both ipg and lead were reprogrammed and remain in use.The patient admitted to having lifted a very heavy trailer in the past and since then had been having chest pain and their family also commented on what a hard worker they were. no further patient complications have been reported as a result of this event.
