Event or Problem Description
The patient was undergoing a coil embolization procedure in the coronary artery using ruby coils and a lantern delivery microcatheter (lantern). It should be noted that the patient¿s anatomy was moderately tortuous and moderately calcified.During the procedure, the physician successfully implanted three ruby coils in the target location using the lantern. While advancing another ruby coil through the lantern, the physician experienced resistance at the mid-shaft of the lantern. While attempting to retract and re-advance the ruby coil within the lantern, the ruby coil unintentionally detached. Therefore, the lantern containing the detached ruby coil was removed from the patient, and the ruby coil was then flushed out of the lantern. The procedure was completed using a new ruby coil and the same lantern. There was no report of an adverse effect to the patient.

Additional Manufacturer Narrative
Evaluation of the returned ruby coil confirmed that the embolization coil was detached.Evaluation revealed that the pusher assembly was fractured, and the pull wire was retracted out of the distal fracture. If the ruby coil is advanced against resistance, damage such as kink may occur.Subsequently, retraction of a kinked pusher assembly may worsen to a fracture, resulting in the embolization coil detachment. The reported patient¿s moderately tortuous and calcified anatomy may have caused resistance during the procedure.Further evaluation revealed a kink on the pusher assembly. This damage is incidental and may have occurred during packaging for return to penumbra.Penumbra coils are visually inspected during in-process inspection and during quality inspection after manufacturing. The manufacturing records for this lot were reviewed and did not reveal any outstanding discrepancies, design, or quality concerns.
