Additional information: d9, g3, h3, h6 h3.Evaluation summary: medtronic conducted an investigation based upon all information received.Thirty-five representative samples were available for eval uation.Visual inspection of the representative samples noted that the breathable pouches showed no breaches or damage, and the needles were properly parked in the retainers.Inspection of the retainers confirmed that the sutures were properly wound with no damage to the retainer.A tactile and microscopic inspection of the sutures was performed, and no abnormalities were observed. The foil pouches were also inspected, and no signs of damage or abnormalities were identified.Functional testing noted that the breathable pouches were opened without any tears in the tyvek packaging, and the sutures were removed from the retainers without difficulty.A simple knot was performed on each suture, and the knot was pulled tight. The suture ends were then loaded onto an instron machine by clamping the ends with cord yarn grips. No suture breaking or fraying was observed during handling or loading. The instron pulled the sutures until they broke, and the breaking point load forces were measured and found to meet ep minimum mean and minimum individual specifications.A total of fifteen sealed samples were split in half, and each half was functionally tested for a total of thirty t est samples.According to the ep procedure for simple knot testing, sutures longer than 75 mm were submitted for two measurements, and shorter sutures for one measurement.Based on the results of the simple knot test, the representative samples were deemed satisfactory. It was reported that the suture broke. The reported issue was confirmed. The most likely cause could not be established from the information available. The manufacturing records for each device are thoroughly reviewed prior to release to ensure that it meets all medtronic quality specifications.A secondary review of the device history records found no potentially contributing factors.Medtronic submits this report to comply with fda regulations 21 cfr parts 4 and 803.Medtronic has made reasonable efforts to provide as much relevant information as is available to the company as of the submission date of this report. This report does not constitute an admission or a conclusion by fda, medtronic, or its employees that the device, medtronic, or its employee caused or contributed to the event described in the report.Any required fields that are unpopulated are blank because the information is currently unknown or unavailable.Medtronic will submit a supplemental report if additional relevant information becomes known.
