Correction to h3(device evaluated by mfg) and h6(device code). The reported event could not be confirmed since the device was returned for evaluation and was observed to be deformed, not broken. The returned screw was visually inspected, revealing notable deformation of the torx recess on the screw head, which appeared completely distorted due to the application of excessive force.Prominent scratch marks were observed on the screw head, along with a shiny wear surface beneath the head. These marks indicate that the sides of the screw contacted the implant plate, suggesting that the screw was inserted under misalignment.A review of the device history for the reported lot did not indicate any abnormalities. No corrective actions are required at this time.A review of the labeling did not indicate any abnormalities. No indications of material, manufacturing or design related problems were found during the investigation.Based on investigation, the root cause was attributed to be user related. If more information is provided, the case will be reassessed.
