Correction to h3(device evaluated by mfg) and h6(device code). The reported event could not be confirmed since the device was returned for evaluation and was observed to be deformed, not broken. The returned screw was visually inspected and showed distinct deformation marks around the screw head.At the torx recess, markings were observed along the edges, indicating that the screwdriver slipped during use.Additionally, a small dent was noted on one side of the screw head.A review of the device history for the reported lot did not indicate any abnormalities. No corrective actions are required at this time.A review of the labeling did not indicate any abnormalities. No indications of material, manufacturing or design related problems were found during the investigation.Based on investigation, the root cause was attributed to be user related. If more information is provided, the case will be reassessed.
