(b)(4).One unit of visistat 35r (p/n 528135, lot 73d2400447) was returned for evaluation following a report of misfire/jamming with misaligned staples.Visual inspection of the returned device identified multiple staples misaligned at the front of the cover block.Further examination revealed that the bottom component was not welded at the back of the cover block, allowing movement of the component and resulting in staple misalignment.Functional testing could not be performed due to the observed staple misalignment.Device disassembly also identified die-casting anomalies on the forming tool.Review of the device history record for the complained lot (manufactured 04/09/2024; released 04/24/2024) did not identify manufacturing nonconformances related to the reported issue. An additional investigation specific to this failure mode was not initiated, as the same failure mechanism had previously been investigated under mdr 3003898360-2024-01396 for the same finished good. That investigation resulted in the opening of a capa. The capa identified the probable root cause as inadequate manufacturing controls associated with the ultrasonic welding process of the bottom component to the cover block, leading to improper positioning of the bottom component.Based on the available information, the reported failure mode is consistent with a previously identified manufacturing issue that is being addressed under the existing capa.No new or different failure mechanism was identified in this complaint. The issue remains subject to ongoing monitoring and trending.Other remarks: n/a corrected data: n/a.
