Event or Problem Description
It was reported that, during an unspecified procedure, after the surgeon finished implanting the twinfix ultra ha 4.5 w/2 ub (wh & blue, the head of the inserter device broke inside the patient's body, with a length of approximately 5mm and 2 pieces. The broken pieces were too small to be found under fluoroscopy; there is possibility of fragments residue inside the patient. The procedure was resumed using the same device, and it is unknown if there was a delay. No further complications were reported.

Additional Manufacturer Narrative
H11: h3, h6: a device deficiency was not identified, and the root cause of the reported event could not be determined since the device was not returned for evaluation.A review of device records showed there were no indications to suggest that the product did not meet manufacturing specifications upon release for distribution.A complaint history review found similar reported events. The instructions for use were reviewed and found to include conditions of off label use and technique specifics, as well as precautions and warnings related to the use of the device.A risk management review found that the reported failure and/or harm was documented appropriately, and there were no indications to suggest the anticipated risk is not adequate.A review of the material specifications found that the storage requirements for the material are specified, and the material must comply with provided percentage composition specifications as measured by ash test.A clinical review states that a definitive clinical root cause could not be determined based on the limited information provided; however, a user technique/procedural variance cannot be ruled out as the ifu cautions against using excessive force which may result in sa and/or inserter failure. The patient impact is determined to be the reported two small, retained fragments of the externally communicating device (inserter). However, micromotion/migration cannot be predicted, and corrosion/local tissue inflammation and/or pain cannot be ruled out as the location of the retained fragments are reportedly unknown. Please refer to the instructions for use for precautions and warnings related to the use of the device.Based on this investigation, the need for corrective action is not indicated. If any additional information related to this event is received in the future, this investigation will be reopened for evaluation.
