The reported event was confirmed capa 8266921 related.Visual evaluation of the returned sample noted one opened (without original packaging), used urine meter drainage bag with an inlet tube, sample port connector, catheter and test.Visual inspection of the sample noted that the catheter will be tested for "deflation due to trauma." using the (in house) syringe the catheter balloon was inflated with 10 ml methylene blue solution (3 drops 1 percent aq methylene blue per 100ml distilled water) and upon inflation ballon was able to inflate but solution leaked from eyelets. The balloon notch was dissected and a 0.025 pin gage fitted within perforation notch.A potential root cause for this failure could be "it was difficult to assure the positioning of the catheter due to vision was difficult".The labeling and risk assessment was not completed as they are addressed by existing capa 8266921.Dhr reviews are not required as the reported event is confirmed capa 8266921 related.Corrections: d, f, h.Udi format updated with available product information per gudid.H11: sections a through f - the information provided by bd represents all the known information at this time.Despite good faith efforts to obtain additional information, the complainant / reporter was unable or unwilling to provide any further patient, product, or procedural details to bd.
