Event or Problem Description
It was reported that they were having trouble getting the pads to fill. They filled the reservoir, restarted therapy, and still are not getting any flow. They are getting fluid delivery line (fdl) open message.Flow rate was 0lpm, inlet pressure (ip) was -0.1psi, circulation pump command (cpc) was 100 percentage, pump hours were 139.1 and system hours were 3845.Stopped therapy.Disconnected pads and started manual mode. With no pads connected, 1.8lpm, -6.8psi, 56 percentage.Connected the left chest pad.0.4lpm, -0.1psi, 100 percentage.Disconnected chest pad.Nurse connected a universal pad.It was not on the patient. They just brought it in to troubleshoot with.0.6lpm, -6.9psi, 36 percentage.Connected left thigh pad.1.3lpm, -7.2psi, 40 percentage.Connected right thigh pad.2lpm, -7.2psi, 53 percentage.Connected right chest pad.2.5lpm, -7.3psi, 65 percentage.Reconnected left chest pad and inlet pressure (ip) and flow dropped to 0.Removed left chest pad ngkr1122 and asked nurse to place it behind the nurse's station for return.Disabled manual mode and started therapy.Explained could use 1-2 universal pads in place of the left chest pad if it offers adequate body surface area coverage, being careful that that patient was not lying on the tubing.Alternatively, replace with another left chest pad.Per follow up information received via task on 12dec2025, spoke with charge nurse who confirmed pads were onsite.Send box addressed to centrastate medical center ccu.

Additional Manufacturer Narrative
The reported event was inconclusive because this investigation did not result in any additional findings and no sample was available for evaluation. The reported event is addressed within the labeling as it state: directions: 2.Select the proper number, size and style pad for the patient size and clinical indication. However, the rate of temperature change and potentially the final achievable temperature is affected by pad surface area, patient size, pad placement and water temperature range.Best system performance will be achieved by using the entire pad set (4).If the entire set of pads is not used, the minimum flow rate may not be achieved.3.For patient comfort, the pads may be prewarmed using water temperature control mode (manual) prior to application.4.Place the pads on healthy, clean skin only.Remove any creams or lotions from patient¿s skin before pad application.Remove the release liner from each pad and apply to the appropriate area. The pads may be overlapped or folded adhesive-to-adhesive to achieve proper placement. The pads may be removed and reapplied if necessary. The pad surface must be contacting the skin for optimal energy transfer efficiency.Place pads to allow for full respiratory excursion.5.Attach the pad¿s line connectors to the patient line manifolds.Begin circulating water through the pads using either patient temperature control mode (automatic) or water temperature control mode (manual). If the pads fail to prime or a significant continuous air leak is observed in the pad return line, check connections, then if needed replace the leaking pad.6.Once the pads are primed, assure the flow rate displayed on the control panel is greater than 1.7 liters per minute, which is the minimum flow rate for a full pad kit (4).A review of the device history record did not show any problems or conditions that would have contributed to the reported issue. No additional actions can be taken at this time.Correction: d, h.Udi format updated with available product information per gudid.H11: section a through f - the information provided by bd represents all the known information at this time.Despite good faith efforts to obtain additional information, the complainant / reporter was unable or unwilling to provide any further patient, product, or procedural details to bd.
