The real intelligence robotic drill, part number rob10013, serial number (b)(6), intended for treatment was not returned for evaluation. An image was provided.A visual inspection confirmed the collets of the drill became disengaged.A complaint history review was performed by part number, and similar complaints were identified.A review by serial number was performed and no similar complaints were identified.A review of manufacturing records indicates the device met all specifications upon release into distribution. The failure mode and associated risk have been anticipated within the risk file, and the documented risk level is still adequate.A historical review concluded that no prior escalation actions are applicable to the scope of the reported complaint. We have no reason to suspect that the product failed to meet any product specifications at the time of manufacture.Although the reported problem was confirmed through a visual inspection, a definitive cause could not be determined.Factors contributing to the reported problem may have been associated to wear and tear.Based on the investigation, no containment or corrective action is recommended or required at this time.Should any additional information be received, the complaint will be reopened. The failure mode will continue to be closely monitored through complaint investigation and trended through post market surveillance activities.

Additional Manufacturer Narrative
Internal complaint reference: (b)(4).H11: the ri robotic drill attachment, part number rob10015, serial number (b)(6), intended for treatment was returned for evaluation.A visual inspection was performed.Drill attachment was still attached to customer returned drill. The exterior showed wear due to normal use.Additionally, the wiper was observed to be widened, and the wiper end showed signs of damage due to use.The reported problem was confirmed with a functional evaluation. The long attachment could not be removed manually from customer drill. The drill was disassembled in order to remove the long attachment.During disassembly, it was observed that the bearing nut on the long attachment was loose.Causing the attachment to become stuck on the drill.Additionally, pieces of the nose assembly were found lodged inside the long attachment. This condition caused the device to fail calibration and generate a system time error message.A complaint history review was performed by part number and similar complaints were identified.A review by serial number was performed and no similar complaints were identified.A review of manufacturing records indicates the device met all specifications upon release into distribution. The failure mode and associated risk have been anticipated within the risk file and the documented risk level is still adequate.A historical review concluded that no prior escalation actions are applicable to the scope of the reported complaint. We have no reason to suspect that the product failed to meet any product specifications at the time of manufacture. The most likely cause of this event is associated with a loose bearing nut due to vibration from use and wear.Based on the investigation, no containment or corrective action is recommended or required at this time.Should any additional information be received the complaint will be reopened. The failure mode will continue to be closely monitored through complaint investigation and trended through post market surveillance activities.
