It was reported that a patient underwent a primary breast augmentation surgery with implantation of a 535cc mentor memorygel breast implant prosthesis.Post-operatively, the patient reported experiencing a ruptured left breast implant.Mammogram, ultrasound, and magnetic resonance imaging were performed.Additionally, the patient stated a breast biopsy is needed to rule out breast cancer.Magnetic resonance imaging indicated a small amount of fluid collection in the left breast without concern.Additionally, the patient was diagnosed with bilaterally ruptured implants and breast ptosis by a medical professional. As a result, the patient underwent bilateral exchange with mentor saline implants on (b)(6) 2025.A post-operative diagnosis of bilaterally ruptured implants was provided; however, the left implant was described as ruptured while the right was described as "appeared to be unruptured".The right-side rupture was not confirmed by the information provided, but the description indicates the surgeon was unsure if the right implant was truly ruptured. This report is for the right breast prosthesis.

The mentor failure analysis lab has received the device for evaluation. The analysis has begun but is not complete at this time. When the investigational analysis has been completed, a supplemental report will be submitted.A manufacturing record evaluation (mre) was performed, and no anomalies were found related to this complaint. In addition, the mre verifies that the device was manufactured in accordance with documented specification and procedures.Reason for device explant and/or reoperation: suspected rupture.Mentor is submitting this report pursuant to the provisions of 21 cfr, part 803.This report may be based on information which mentor has not been able to investigate or verify prior to the required reporting date. This report does not reflect a conclusion by fda, mentor, or its employees that the report constitutes an admission that the device, mentor, or its employees caused or contributed to the potential event described in this report. If certain information is unknown, not available or does not apply, the section/field of the form is left blank.Manufacturer¿s reference number: (b)(4).

On december 26, 2025, mentor completed an evaluation on the returned device.Device evaluation: mentor conducted visual inspection and leak testing of the device.During visual evaluation no apparent damage or visual anomalies were observed on the returned device.Leak testing was performed, in accordance with mentor procedures, and no areas of gel exposure were detected during the analysis.Although no conclusion could be reached on the cause of the reported event, the instructions for use contain the following caution: rupture status identified by mri evaluation includes both suspected ruptures, which are those ruptures identified by mri but not confirmed by explantation and examination of the device, and confirmed ruptures, which are those ruptures that are confirmed by evaluation of the explanted devices. The event described could not be confirmed as the breast implant was returned without detectable gel exposure.Although no product defect was identified, there may have been other circumstances or issues that occurred during the use of the device that could not be replicated during the laboratory analysis. Some breast ptosis is a normal component of the mature breast.Ptosis becomes undesirable when the breast parenchyma predominates below the areola, droops considerably below the inframammary fold and the nipple points downward.Augmentation alone, without consideration of the ptosis can produce a less than desirable cosmetic result known as a ""rock in sock"" deformity or an increased ptotic appearance.Ptosis is a known complication associated with these devices and is referenced in our current product insert data sheet.Manufacturer¿s reference number:(b)(4).
