Correction: upon further review, it was noted that lens damage - device code: 1069 reported in the initial mdr was incorrect and no reported device problem - device code: 2993 was more appropriate.Iol removal and replacement and device decentered/dislocated codes were also found to be appropriate for the complaint.Additional information: additional information was received confirming the iol was fully inserted and the vitrectomy was unplanned. There was no incision enlargement, no sutures, and the same procedure was completed using a non-johnson & johnson surgical vision lens, ma60ac, that was implanted in the patient¿s ocular dexter (right eye). There was no product quality issue that contributed to the event. No further information is available. The corrected information is captured in this supplemental report. The following fields were updated accordingly: section a-3b: patient gender: male.Section a-4: patient weight: 195 lbs.Section h-6: health effect - device code: 2993 - no reported device problem.Section h-6: health effect - impact code: 4631 - iol removal and replacement.Section h-6: health effect - clinical code: 4581 - device decentered/dislocated.All pertinent information available to johnson and johnson surgical vision, inc.Has been submitted.
