It was reported that a patient underwent a primary breast augmentation surgery with implantation of a 430cc mentor memorygel breast implant prosthesis.Post-operatively, the patient experienced right breast soreness after exercising. She observed the right breast became flat and the nipple/areola were lower than on the left.During a physical exam, she was noted to have a smaller right breast which was firm and mildly constricted/flat in the superior pole.A diagnosis of possible right breast capsular contracture (no baker grade was provided) or a ruptured right implant was provided. As a result, the patient underwent bilateral exchange with mentor gel implants on (b)(6) 2025.During the surgery, a ruptured right implant was confirmed.

On january 07, 2026, the mentor analysis lab received the suspect medical device for evaluation. On january 14, 2026, mentor completed an evaluation on the returned device.Device evaluation: mentor conducted visual inspection.Visual analysis of the returned device revealed that the breast implant was found to be ruptured. The evaluation determined that the possible cause of the rupture is the trauma experienced.Rupture can occur at any time after implantation, but it is more likely to occur the longer the implant is implanted.Trauma is a known inherent risk of gel-filled mammary prosthesis.Regarding the reported capsular contracture condition, there are not enough details to determine the factors that may have caused this condition.Capsular contracture in the patient¿s breast might be the result of the body¿s individual physiological response to the implantation of a foreign object in soft tissue.Further, capsular contracture is a known complication associated with these devices and is referenced in our current product insert data sheet. On january 19, 2026, mentor was provided with the baker grade rating. The patient was diagnosed with right breast baker grade iii capsular contracture.Manufacturer¿s reference number: (b)(4).
