The reported adverse event/incident was investigated in alignment with endologix operating procedures and work instructions. An evaluation of the manufacturing record was completed.A review of the part number/lot number combination needed for device identification shows the device to have been properly manufactured and released in accordance with the device master record. An evaluation of the device could not be completed. The device was not returned to endologix for evaluation because it remains implanted.A clinical evaluation of the adverse event/incident was completed. An examination of medical records and/or medical imaging received by endologix shows the type 1a endoleak and additional endovascular procedure are confirmed. This is consistent with the reported adverse event/incident.The complaint is likely user and anatomy related. No procedure related harms were identified. The clinical evaluation also shows reasonable evidence to suggest there is progressive aortic degeneration from a preexisting infection (mycotic aneurysm) likely contributed to the type ia endoleak which is anatomy related. At the index procedure, the patient was being treated for a ruptured mycotic aneurysm with a colonic fistula.Use of the afx device to treat a mycotic aneurysm is considered off label, as the device is not designed or cleared for infected aortic tissue and is contraindicated in patients with a condition that threatens to infect the graft. The patient experienced blood loss with a hemoglobin of 5, requiring postoperative packed red blood cell transfusion. The blood loss anemia was likely preexisting secondary to a gastrointestinal bleed and was unrelated to the endovascular procedure. The surgical colectomy was performed for a preexisting abscess/infection and colonic fistula and was not device related. The final patient status was reported as discharged to skilled nursing facility in stable condition on postoperative day 11.No additional investigation of this reported adverse event/incident is planned. However, should additional information relevant to the investigation outcome become available, a follow-up report will be submitted.Endologix will continue to monitor this and similar adverse events/incidents.
