It was reported that the procedure was to treat a lesion in the left superficial femoral artery (sfa) with no tortuosity. The 6x100mm abs pro vascular self-expanding stent system (sess) was advanced on a 0.14 viper guide wire (gw) through a 6f sheath to the target lesion; however, the thumbwheel would not turn and as a result the stent completely failed to deploy. The sess was removed from the patient.Another same size abs pro vascular sess was advanced on the same gw with the same sheath.During deployment of the second sess, the thumbwheel was sticking and difficult to turn; however, the stent was ultimately able to be deployed. There was no adverse patient effect and no clinically significant delay reported in the procedure. No additional information was provided.

The device was returned for analysis. The reported physical resistance / sticking and the reported difficult or delayed activation were unable to be replicated in a testing environment due to the condition of the returned device (stent previously deployed).Production record and corrective and preventive actions (capa) reviews were performed and revealed no indication of a product quality issue.Additionally, a query of the complaint handling database for the reported lot revealed there is no indication of a lot level product quality issue. As there was no damage noted to the device during the inspection prior to use, the investigation determined the reported difficulties appear to be related to deviation of the instructions for use and subsequent circumstances of the procedure as it is likely that use of the undersized 0.014¿ guide wire resulted in the noted multiple stent sheath kinks preventing the shaft lumens from moving freely; thus resulting in the reported physical resistance / sticking and the reported difficult or delayed activation. It should be noted the absolute pro self-expanding stent system instructions for use states: one (1) 0.035" (0.89 mm) diameter guide wire.Use of an undersized guide wire, with insufficient support, may cause kinking in the stent delivery system. As the deviation of the instructions for use likely caused/contributed to the reported difficulties an in-service letter to the physician will be required.Based on the reported information and results of the complaint investigation there is no indication of a product quality issue with respect to the design, manufacture, or labeling of the device.
