Event or Problem Description
Medtronic received a report that a pipeline failed to open in the distal section. The patient was undergoing surgery for treatment of a saccular aneurysm of the right internal carotid artery with a max diameter of 5mm and a 5mm neck diameter. The landing zone was 4.7mm distally and 4.9mm proximally. It was noted the patient's vessel tortuosity was normal. The angiographic result post procedure was good and the case went well.It was reported that while trying to unsheath/deploy the distal end of the device, the device would not open. The physician attempted to recapture/resheath the device and the distal end of the device would not capture inside the microcatheter. Both the device and microcatheter were removed and a new pipeline device was used with a new microcatheter with no issues. The pipeline was not positioned in a bend.Less than 50% of the pipeline had been deployed when it failed to open. The pipeline was resheathed less than or equal to 2 times. There were no additional step or other devices required to open the pipeline. The pipeline was used for an indication that is approved (on-label).The reported device and any accessory devices were prepared as indicated in the instructions for use (ifu). No patient symptoms or complications were associated with this event.
