The device was returned to olympus for inspection.Based on the results of the investigation, the distal end has corrosion and the switch 4 sticky is likely the result of insufficient reprocessing; however, a definitive root cause could not be established.Should additional relevant information become available, a supplemental report will be submitted.Olympus will continue to monitor field performance for this device.

It was observed that during the device evaluation, the duodenovideoscope presented corrosion on distal end and sticky switch 4.There was no patient involvement.
