Model number/catalog number: 720182-01.Serial number: n/a.Batch/lot number: 0190538010.Model/catalog description: reservoir.D4 unique identifier (udi): (b)(4).Model number/catalog number: 72404310.Serial number: n/a.Batch/lot number: 1100383445.Model/catalog description: pump.D4 unique identifier (udi): (b)(4).Updated concomitant product comment.Upon receipt at our quality assurance laboratory, this device underwent a thorough analysis. The cylinders, pump and reservoir were microscopically examined, and leak tested; the pump was also functionally tested.Testing of the first cylinder revealed a large hole in the inner tube, broken fabric threads from fatigue, and wear at folds in the proximal, middle, and distal ends.Additionally, the outer sleeve was detached at the proximal end from wear, and a bulge was observed due to excess air between inner and outer tubes when inflated with syringe. The first cylinder did not pass leakage testing due to leaks from fatigue in the proximal end.Regarding the momentary squeeze (ms) pump, the tubing was cut down to the pump block; the tubing for the first cylinder was returned attached, while the tubing for the second cylinder and reservoir was not returned for analysis. The ms pump passed leakage and functional testing.Testing of the second cylinder revealed that it passed visual inspection and leakage testing. The device history record (dhr) review confirmed that the device met all material, assembly and performance specifications.A review of the device instructions for use (ifu) was completed and did not reveal any evidence of device misuse, off label use, or failure to follow instructions.A risk review was completed and confirmed that the event of device has a fluid leak and mechanical malfunction (leaks, incomplete inflation/ deflation, kinking) was defined in the product risk documentation. This event type has been accounted for during analysis to support acceptable risk benefit for the product.Based on the information available and analysis results, the hole and leak identified in one of the cylinders could have caused or contributed to the reported clinical observation of mechanical problem.
