H11: section a through f - the information provided by bd represents all of the known information at this time.Despite good faith efforts to obtain additional information, the complainant / reporter was unable or unwilling to provide any further patient, product, or procedural details to bd.The complaint of difficult obturator removal following needle placement was inconclusive because the returned products were unused and were therefore not the originally implicated/used device. The product returned for evaluation was three 15mm intraosseous (io) needles. All three samples were received in their original sealed packaging.Visual inspection of the needle/obturator assemblies was unremarkable.During manipulation, the needle luers felt slightly loose within the obturator hubs.Removal of the obturators was successful and unremarkable. No resistance was encountered.Needle and obturator hub features were measured using a digital microscope and a top-down viewing angle. Those measurements were compared against the specifications.Several dimensions were found to be outside of manufacturing specifications. These out of specification dimensions are addressed in trackwise complaint (b)(4).The returned samples were sealed, indicating they were not the products used in the field. While features of the returned samples were found to be outside of manufacturing specifications, it is unknown if the implicated device exhibited similar characteristics. It is further unknown if such features contributed to the reported event.Out of tolerance components may contribute to difficult obturator removal; however, insertion techniques have also been demonstrated to contribute. Those factors include inconsistent/excessive force applied while drilling and insertion at an oblique angle. The product ifu indicates that moderate, steady pressure at a 90-degree angle should be used during the insertion process. It further instructs the user to avoid rocking or bending during insertion. It is ultimately unknown if product features contributed to this reported event and to what extent difficult component separation may have contributed to the failure of resuscitation efforts.
