Event or Problem Description
It was reported that a clearlink continu-flo solution set with duo-vent spike was leaking out of the blue piece on the filter just below the spike after bag was spiked.It was unknown if a patient was connected during this event.There was no report of patient injury or medical intervention associated with this event.No additional information is available.

Additional information was added to h6.H11: the actual device was not available; however, a companion sample was received for evaluation.Visual inspection did not identify any abnormalities that could have contributed to the reported condition.Pressure testing was performed, and it was noted that there was a leak on the edge of the blue part of the chamber.The reported condition was verified.The cause of the reported condition was a manufacturing issue.A batch review was conducted and there were no deviations found related to this reported condition during the manufacture of this lot.Should additional relevant information become available, a supplemental report will be submitted.
