As reported, a 5f railway sheathless access system vista brite tip bare wire broke off in the patient¿s body.An additional interventional radiology procedure was required, which involved snaring the device fragment out of the body via femoral approach. There was no additional reported patient injury. The access site vessel was not calcified but was described as severely tortuous. The device was flushed prior to use, and the vessel dilator was snapped into place at the hub.Difficulties during insertion were attributed to a small vessel with severe tortuosity. There were no difficulties withdrawing the device; however, upon withdrawal, a fragment was left behind after attempting to advance the system through severe tortuosity, and repeated torquing during insertion resulted in fracturing and shearing off the dilator. No damage was noted on the device or packaging prior to opening. The device was stored and prepared according to the instructions for use (ifu), the user was trained on the device, and the patient was reported to be stable and discharged. The device will not be returned for evaluation as all products from the procedure were discarded.
