H6, h11: the device was not returned for evaluation; therefore, a device analysis could not be performed. However, the clinical/medical investigation concluded that, the fluoroscopic images appear to show the targeting in the most superior aspect of the proximal lag screw hole as well the introduction angle doesn¿t appear consistent with the lag screw hole which would support the complaint. The explanted nail shows scratches/deformation of the proximal lag screw hole likely from attempted lag screw targeting/implantation, as well as the proximal nail end, likely due to extraction. The instructions for use for trigen¿ intertan¿ nail system reveals that it is the healthcare professional¿s responsibility to be familiar with the appropriate surgical techniques prior to use of this device proper surgical technique and implant selection are essential to a successful outcome. The surgical technique includes specific recommendations from assembly to implantation and notes to confirm the nail and drill guide handle are securely connected after definitively seating the nail as hammering can loosen the guide bolt.Nail assembly notes that an incorrectly attached nail will not target.A review of the production order did not reveal a manufacturing abnormality that could have caused or contributed to the reported incident.A review of complaint history for the part number over the past 12 months and for the batch number based on historical data of the device did not reveal similar events for the listed device.A review of the risk management file revealed this failure mode was previously identified. The anticipated risk level is still adequate.A historical review concluded that there are no prior actions related to this product and event. At this time, we have no evidence to conclude that the product failed to meet any specifications at the time of manufacture.Factors that could contribute to the reported event include surgical technique used, size selected or user/procedural variance.Based on this investigation, the need for corrective action is not indicated.Should the device or additional information be received, the complaint will be reopened. No further investigation is warranted for this complaint; however, we will continue to monitor for future complaints and investigate as necessary. We consider this investigation closed.
