It was reported that the cylinder hose exhibited missing o-ring and the unit was leaking at the connection point.The event occurred during service maintenance.There were no reports of patient involvement.

This supplemental report is being submitted to provide the results of the final investigation.The device was not returned to olympus for inspection and the reported failure was not confirmed.Based on the results of the investigation, and since the device was not returned for evaluation, the definitive root cause of the reported issue could not be determined.Should additional relevant information become available, a supplemental report will be submitted.Olympus will continue to monitor field performance for this device.
