Block h11: investigation results the returned resolution clip device was analyzed, and during visual inspection, it was found that the device was returned with the clip assembly detached from the bushing but attached to the control wire and with evidence of soft deployment.Additionally, the outer sheath was not returned.Microscopic evaluation was performed, and it was noted that the first activation was not performed.Media analysis was performed, and the photo provided shows the device with evidence of soft deployment. No other problems with the device were noted. It was confirmed this device met manufacturing specifications prior to distribution, and there were no manufacturing deviations which could have contributed to the reported event. The reported event of clip partial release was confirmed.Investigation found that the device was returned with the clip assembly detached from the bushing but attached to the control wire and with evidence of soft deployment. Also, there is a picture confirming the analyzed condition.Regarding the soft deployment, it may have been caused by the insertion of the device into the scope, the physician's technique, or any unrecorded impacts to the joint during preparation. The joint capsule bushing might have detached due to an external force impacting this joint.Based on the thorough review of the reported complaint, boston scientific has assigned an investigation conclusion code of adverse event related to procedure.
